FMIC will not provide you with one, unless the instrument is verified as a genuine Fender guitar by a Fender Approved Dealer - the dealer will then order the decal for you. For obvious reasons, the company don't want their proud name put on a fake!
You will have to do a search on the Net for someone who is willing to create a copy, or make one up to your own design.
